At Healing House LLC, we make the admission process simple, compassionate, and collaborative.
Our goal is to ensure a smooth transition from hospital or shelter care into our recuperative program—so individuals can focus on what matters most: healing and recovery.
🌿 Step 1: Referral Submission
Healing House LLC accepts referrals from hospitals, clinics, shelters, and community agencies.
Referrals can be submitted by completing our Recuperative Care Referral Form (available online or by email).
Referral information should include:
Client’s name, age, and contact information
Recent medical diagnosis or reason for hospitalization
Current medications and care needs
Expected recovery timeframe
Insurance or funding source (if applicable)
Discharge summary and provider contact

🩺 Step 2: Clinical Review
Once we receive the referral, our nursing and intake team reviews the information to confirm eligibility for recuperative care.
We coordinate directly with hospital discharge planners or medical providers to ensure that the guest’s condition is stable and appropriate for a residential setting.
If additional information is needed, our intake coordinator will reach out to clarify medical needs or discharge details.
🏡 Step 3: Admission Coordination
After clinical approval, our team will:
Confirm the guest’s acceptance into the program
Schedule an admission date and arrange transportation (if needed)
Prepare a room and welcome packet for the guest’s arrival
Guests receive an overview of program guidelines, available services, and daily routines upon admission.
💙 Step 4: Welcome & Orientation
Upon arrival at Healing House LLC:
A nurse conducts an initial assessment (vitals, medications, wound care if applicable)
A case manager introduces available resources such as housing, benefits, and follow-up appointments
Guests are welcomed by staff and given a tour of the facility
Our team ensures every new arrival feels safe, respected, and supported from day one.

Once admitted, guests receive:
A personalized recovery plan
Ongoing medical and emotional support
Connection to community and housing resources
